Medication Reconciliation

This is a shorter visit focused on medication changes after a hospital stay.  We want to be sure you understand any changes and how to safely take your new meds.

Our services are conducted in person, especially for the initial visit.  Follow up visits may be available via Telehealth on a case-by-case basis.
